Job Title: Locum Nurse (For a 78 years Old Man)

Job Summary
- The Client is still very agile and has his full motor skills.
- He however presents with signs of dementia.
- The Locum Nurse provides temporary, high-quality, one-on-one medical and personal care to an elderly patient at home.
- They are responsible for implementing care plans, administering medication, monitoring health changes, and offering emotional support, ensuring the patient remains safe and comfortable in their familiar environment
- The Nurse will is expected to provide weekly report to the family.
Key Responsibilities & Duties
Medical Care and Monitoring:
- Vital Signs Management: Regularly track and record vital signs (blood pressure, temperature, heart rate, oxygen levels) to identify changes early.
- Medication Administration: Manage and administer prescribed medications, including oral, injection, or IV, and monitor for side effects.
- Hospital visits: schedule hospital visits, chaperone to hospital and provide updates to family
- Diagnostic Procedures: Conduct basic lab tests as needed, such as blood sugar checks, and take relevant samples to the laboratory- urine sample, blood sample
Daily Living and Physical Support:
- Supervise cleaning staff: Ensure that the cleaning staff keeps the house clean and ventilated at all times.
- Activities of Daily Living (ADLs): Assist with reminders on hygiene practices eg Brushing of teeth, change of clothes, etc
- Mobility Support: Assist with exercise programs to support muscle strength and prevent falls.
- Nutrition Management: Supervise or prepare meals that meet nutritional needs and address issues like swallowing difficulties.
Safety and Care Management:
- Safety Assessments: Maintain a safe environment by identifying and removing fall hazards (e.g., rugs, cords).
- Care Plan Implementation: Execute treatment plans established by physicians and collaborate with family members and other healthcare providers.
- Documentation: Maintain accurate, up-to-date records of all care provided and patient status.
Emotional and Social Support:
- Compassionate Care: Build a trusting relationship, offering companionship to reduce loneliness and depression.
- Dementia/Memory Care: Support patient with memory exercise; manage behavior problems, confusion, and anxiety with patience and expertise.
Qualifications and Skills
- License: Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) in good standing or Health Technologist / Nurse Assistant
- Experience: Minimum of 2 years in geriatric, home health, or medical-surgical nursing.
- Certifications: CPR/First Aid certification required. Gerontological Nursing Certification is highly preferred.
- Key Skills: Strong clinical assessment skills, wound care proficiency, and medication management expertise.
- Soft Skills: High levels of empathy, patience, adaptability, and the ability to work independently without supervision.
Typical Work Schedule:
- Flexible, at least 3 days a week, and may include weekends, evenings, and may support overnight stay where necessary
