CHANGE OF NAME 

  • Change of name is allowed in the records of the Council however some conditions must be met in order for the name to be legally changed by the Council. 
  • There are five (5) reasons for change of name as approved by the Council each of which has requirements to be fulfilled. 
  • Change of Name in the Council attracts the payment of #8,750 
  • All applications for change of name are treated based on merit

REASONS FOR CHANGE OF NAME 

  • First time marriage 
  • Divorce and reversal to maiden name 
  • Divorce and new marriage 
  • Death and reversal to maiden name 
  • Death and new marriage

PROCESS OF ONLINE APPLICATION FOR CHANGE OF NAME 

  • Initiate online application for change of name on the Council's licencing portal https://licence.nmcn.gov.ng
  • Upload scanned copy of requirements as prompted based on the applicants reason for change of name 
  • Payment of #8,750 only via Remita using the Remita Retrival Reference (RRR) generated on the portal.

FIRST TIME MARRIAGE 

Requirements are as follows: 

1) Marriage Certificate of Affidavit of marriage 

2) Affidavit of Marriage if names are being compounded 

3) Newspaper Publication in a National daily

DIVORCE AND REVERSAL TO MAIDEN NAME 

• Requirements are as follows: 

1. Divorce papers 

2. Marriage Certificate or Affidavit of previous marriage 

3. Newspaper publication in a National daily for reversal to maiden name

DIVORCE AND NEW MARRIAGE 

Requirements are as follows: 

  1. Marriage certificate or Affidavit of marriage to the first husband 
  2. Divorce papers 
  3. Marriage certificate or Affidavit of marriage to the new husband 
  4. Newspaper publication in a National daily

DEATH AND REVERSAL TO MAIDEN NAME 

Requirements are as follows: 

1. Marriage certificate to the deceased 

2. Death certificate 

3. Newspaper publication in a National daily for reversal to maiden name

DEATH AND NEW MARRIAGE 

Requirements are as follows: 

1. Marriage certificate to the deceased 

2. Death Certificate 

3. Marriage Certificate or Affidavit of marriage to the new husband 

4. Newspaper publication

GENERAL REQUIREMENTS 

  • Compound names must be hyphenated in all the attached documents 
  • Sworn affidavit of marriage should be attached in addition to marriage certificate when applicant is requesting for compound name 
  • Full page of Newspaper publication should be attached 
  • Newspaper publication for change of name should be in a National Daily 
  • Sworn affidavit of marriage should be attached if Marriage certificate is not available

REASONS FOR REJECTION OR DISAPPROVAL OF CHANGE OF NAME APPLICATION

Applications for change of name can be rejected or disapproved as a result of the following reasons: 

  • Incomplete requirements 
  • Change of first or middle name 
  • Change of name based on cultural or religious reasons
  • Wrong certificate 
  • Incomplete page of newspaper publication 
  • Publication for change of name made in a periodic print media and magazines 
  • Uploading a sworn affidavit of change of name in place of affidavit of marriage 
  • Uploading documents with unhyphenated names for applicants seeking change to compound names e.g Adeniyi-Adamu, Okonkwo-Ayedun

ISSUES ON DATE OF BIRTH 

  • The Council strongly recommend that any date of birth provided at the point of indexing during training should be maintained through out the course of a Nurse’s career. 
  • It is in fact the policy of the Council that the date of birth provided by the student should be used on all the documents issued by the Nursing and Midwifery Council of Nigeria for example licence and verification 
  • Hence no Nurse or Midwife is expected to request for a change of date of birth
  • However, issues surrounding disparities in date of birth of names may be attended to by the Council by writing to the Secretary General/Registrar of the Council 
  • Evidences of discrepancies should also be attached to the request 
  • Requests of this nature are scrutinized and treated based on merit
