1. Certificates of Academic Qualification 
2. Annual Practicing License
3. Birth Certificate / Declaration Age
4. Certificate of Identification of State of Origin
5. Detailed Curriculum Vitae stating all personal information including state and Local Government of Origin. 

Application should be addressed to the Chief Medical Director. University of Abuja Teaching Hospital, Gwagwalada. Closing date for submission of application is 20th August, 2021 by 12noon.
