The Honourable Minister for Health has mandated the Nursing and Midwife, Council of Nigeria to compile a list of all health facilities in Nigeria and the personal profile of registered Nurses and midwives employed in those facilities.

It is hoped that information obtained from this survey will be utilized to develop a National Health Registry and a basis to conduct a license verification exercise of all nurses in the country.

In view of the above, I am directed to inform you to liaise with all State Directors of Nursing Services, Directors of Nursing Services of State Hospitals Management Boards and other stakeholders in your zone and compile a comprehensive list of all nurses under the employment of Federal, State, Local Government, Nursing Training Institutions and Private own health institutions. The survey is expected to be conducted between 1st to 31^st March, 2019

Attached is a copy of the survey work sheet for distribution to State DNS and Heads of Nursing services in all health facilities in your zone (Public or Private). The forms should be duly completed by all registered Nurses.

Please note that Heads of Nursing services in all health facilities should append their full names and signatures to each sheet bearing the list of nurses and midwives.

All Zonal officers should submit this information in both hard and soft copies to the Council at the end of the exercise.
