OCTOBER 2014 PRIMARY, PARTS I & II (FINAL) FELLOWSHIP EXAMINATIONS
The October 2014 Fellowship Examinations of the west African College of Surgeons (WACS), in the Facultles of Anaesthesia, Dental Surgery, Obstetrics and Gynaecology, Ophthalmology, otorhinolaryngology, Radiology and Surgery, will be hold from 15th to 29th October, 2014. 
THE PRIMARY EXAMINATIONS WILL HOLD ON FRIDAY 17th OCTOBER, 2014 IN FIVE CENTRES AS FOLLOWS:
• University College Hospital (UCH), Ibadan, Nigeria 
• National Centre for Women Development (NCWD), Abuja, Nigeria 
• Korle-Bu Teaching Hospital (KBTH)f Accra, Ghana 
• National Orthopaedic Hospital (NOH) Main Hall, Enugu Nigeria 
• Connaught Hospital. Freetown, Sierra Leone 
THE PARTS I & II EXAMINATIONS WILL HOLD FROM 15th OCTOBER 2014 (Obstetics & Gynaecology) and 20th October 2014 (Anaesthesia - including the Diploma in Anaesthsia, Dental Surgery, Ophthalmology, ORL, Radiology and Surgery) in Two Centres namely:
• University College Hospital (UCH), Ibadan, Nigeria  
• Korle-Bu Teaching Hospital (KBTH)f Accra, Ghana
REQUIREMENTS
• Candidates applying to sit the Diploma in Anaesthesia Examination must have completed the requisite twelve months training in Anaesthesia in an acredited instition. Application must be submitted with APPROVED LOG BOOK/PORTFOLIO and copies of appropriate relevant document.
• Candidates applying to sit the Primary Fellowship Examinations must be medical Practioners who are fully registered in member Countries, and have acquired at least one year Post-registration experience. Candidates must submit their degree certificates and evidence of FULL registration with Medical & Dental Council of their Council of their countries. Provisional Registration Certificate is NOT Acceptable.
• Candidates applying to sit the Part I Fellowship Examination must have passed the Primary Examination of the west African College of Surgeons or its equivalent and satisfactorily completed appropriate years of Postgraduate training as prescribed by the relevant WACS faculty in the insitutions accrdited for the purpose by the College. Candidates are required to submit their original Certificates of Training (COT) form with dates of commemcement and completiong of training clearly stated. 
Altered or defaced COT will not be accepted. Submission of photocopies of log books duly certified by their training Consultants is compulsory  ..Surgeon in Training Registration evidence and Attendace Certificates of relevant Update course(S) are required 
• Candidates Candidates applying to sit Surgery Part I Examination in the Faculty of Radiology must indicate the specialty of interest i.e ( RADIODIAGNOSIS OR RADIOTHERAPY) as it appears in item No 6 on the application form
• Candidates applying to sit the Part II (Final Followship examinations must have passed the Part I Fellowship examinations of the West African College of Surgeons and satisfactorily completed appropriate years of Postgraduate training in their specialty in Institution(s) accredited for the purpose by WACS as prescribed by the relevant Faculty. Candidates are to submit their original Certificates of Training (COT) Form with dates of commencement. completion of training and various posting/appointments clearly stated. Altered or defaced COT will not be accepted 
• Subspecialty as it appers on item no 6 in the application should be completed (this is applicable to Part II Dental Surgery: Radiology & Surgery)
• Candidates are required to submit three (3) Copies of their Thesis/Dissertations with the application before the close of application. 
• Candidates applying to sit Surgery Part II Examinations must submit Three (30 copies of their dessertations and 3 copies of approved Proposal separately bound in spiral format and a copy of their residents log-book at the time of submission of applications for the examination. They should in addition bring their file(s) of the raw results of the dissertation with them to the venue of the Oral examination
• It is the candidates's respomsibility  to retrieve both his/her log-book and disseration at the end of the end of the examnation. A copy of the dissertation shall be kept by the College
• CANDIDATES WHO WISH TO SIT APRIL 2015 PART II EXAMINATION IN O&G MUST SUBMIT CASEBOOKS AND DISSERTATIONS LATEST BY FRIDAY, OCTOBER 3, 2014
• Candidates appluing to sit Dental Surgery must submit Two (2) copies of their logbooks while Part II Candidates for Anaesthesia, ORL and Radiology and are to submit a photocopy of  their Logbooks, Three (3)  copies of Approved Proposal and Three (30 copies aof Dissertations before the closing date of applications. Original copies of logbooks should be brought to the examination venues.
• All Part II candidates must submit an evidence of attendance of Manuscript Writing Workshop.

ALL APPLICATION FORMS MUST BE SUBMITTED WITH
• Original cOPY of Bank Teller - Primary/Part I/Part II
• Copies of Dissertations and approved Proposal - Part II
• Relevant certificates and photocopy of Log Books (PartsI & II} 
• Evidence of attendance of Manuscript Writing Workshop (Part II)
• Evidence of registration as Surgeon in Training (S.I.T) for Part I only
• 3 self addressed envelopes with surficient postage stamps

Current application forms are available at the College Secretariat in Yaba, Lagos and WAPMC Liaison Office at UGMS, Korle Bu, Accra, Ghana. Application forms can also be downloaded from the College website: www.wacs-coac.org
ALL APPLICATION FORMS MUST BE ACCOMPANIED BY "DOCUMENTS INCLUDED CONTROL FORM' WHICH COULD BE DOWNLOADED FROM THE WEBSITE 
ALONG WITH THE APPLICATION FORM AND RELEVANT DOCUMENTS.
DEFERMENT OF EXAMINATION AFTER SUBMISSION OF APPLICATION OR REFUND OF EXAMINATION FEE AFTER PAYMENT IS NOT ACCEPTABLE

THE COLLEGE SHALL BEAR NO RESPONSIBILITY FOR PAYMENT INTO WRONG ACCOUNTS. CASH PAYMENT OR BANK DRAFT IS UNACCEPTABLE. 
Candidates must Indicate their names in the Teller Column 'Paid By'.
Candidates should write their Faculty and Part of the Examination on the TELLER and submit same with their Examination 
Application Form to the College Secretariat either in person or by COurier Only 
CLOSING DATE
The closing date for the submission of compleated application forms is July 18, 2014. Candidates for the Part II (final) examination in Anaesthesia, Dental surgery, Ophthalmology, ORL, Radiology, and Surgery must submit their dissertations by the closing date. NO LATE SUBMISSION WILL BE ACCEPTED.
Applications and Dissertation submitted later than July 18, 2014 will not be processed.
 
NOTE: Any candidate that does not meet all the requirements as at the closing date will not be allowed to sit the examinations.
