Job Title: Nursing Care Coordinator
Location: Remote
Employment Type: Full-time / Permanent
Role Overview
- As a Nursing Care Coordinator, you will be responsible for coordinating care for patients, especially those with complex or long-term conditions, managing care schedules, liaising with clients and carers, and ensuring the highest standards of care are maintained.
- You will play a vital role in ensuring our clients receive timely and effective support while maintaining compliance with regulatory requirements.
Key Responsibilities
- Coordinate and navigate care across the health and social care system, connecting patients with the right teams and services at the right time to enable the delivery of person-centred care.
- Conduct comprehensive risk and needs assessments and develop personalized care and support plans based on what matters to patients.
- Ensure efficient allocation of care staff to meet patient needs and manage care schedules for clients using digital care management system.
- Work with GPs, other healthcare professionals, and community services to ensure a coordinated approach to patient care.
- Maintain health and social records of clients, review care notes, and ensure care plans and risk assessments are updated as needs change using digital care management system.
Requirements
- Bachelor of Nursing qualification.
- Previous experience in a care coordination or adult nursing role.
- Strong organisational and communication skills.
- Ability to work under pressure and manage multiple tasks.
- Proficiency in scheduling software and Microsoft Office.
- A caring and compassionate approach to work.
Other requirements:
- Must have completed NYSC within the last 3 years
- Must be available to work full time(Mondays -Fridays) with Rotational out of office activities.
- Available for immediate start
Salary
- N200,000 - N300,000 / month
