Job Title: Integrated Service Nurse

Job Description

  • The Integrated Service Nurse will facilitate the integration of comprehensive HIV/TB and other health services into existing outpatient, inpatient, and specialized care units, ensuring holistic and client-centered care delivery across supported facilities.
  • They will coordinate the activities of other ancillary staff for HIV service delivery, under the guidance of the TA clinical services and site staff at the health facility.

Responsibilities

  • Support site level activities to embed HIV services into all relevant clinical units (outpatient, inpatient, antenatal, etc.).
  • Ensure seamless coordination between HIV/AIDS services and general health services.
  • Support the delivery of HIV testing services, adherence counselling, and ART initiation at facility and community service delivery points by connecting them to the relevant service providers within the facility or via facilitated referrals, to sustain treatment.
  • Facilitate diagnosis and treatment of co-infections and comorbidities.
  • Support PMTCT activities, including antenatal, intrapartum, and postnatal care.
  • Coordinate the delivery of health education and treatment literacy to PLHIV on relevant topics including ART, treatment adherence, U=U, and positive living.
  • Ensure service alignment and timely uptake of relevant services (e.g. TPT, MMD, DSD, Viral Load) to maximize client encounters, reduce service fragmentation and improve treatment outcomes.
  • Oversee pre-appointment reminders and tracking of missed appointments within 24 hours via phone, SMS, or home visits to ensure treatment continuity.
  • Support in the delivery of enhanced adherence counselling for clients with unsuppressed viral load.
  • Facilitate the uptake of health insurance enrollment for PLHIV in assigned health facilities
  • Perform additional duties as assigned.

Minimum Recruitment Standard

  • Bachelor’s Degree in Nursing, or equivalent.
  • Current practicing license
  • Strong knowledge of HIV care, treatment, and prevention services.
  • Coordination and supervisory skills.
  • Effective communication skills in English and the local dialect.
  • Basic computer skills, including proficiency in Microsoft Word, PowerPoint and Excel.
  • Ability to work in multidisciplinary teams and manage competing priorities effectively.
