The sale of CHO application from commence on 6th June - 12th July, 2022. The price of the form is ten thousand ( 10,000.00) naira only. 

ELIGIBLE CANDIDATES: 

The following category of candidates are eligible for admission; 

1. Holders of Diploma in Community Health or Community Health Extension Workers with first degree in health related course in an accredited University. 
2. Holders of Diploma in Community Health with Bsc in Community Health. 
3. Must posses current practicing license from Community Health Practitioners Registration Board of Nigeria. 
4. Candidate with above certificates must possess five (5) credits in English, Mathematics, Biology, Chemistry and Physics in SSCE/0 level not more than two (2) sittings.

All payment should be made into School account through billing office UMTH remita platform. Candidates is to present the original and photocopy of evidence of payment to School cashier and obtain hospital receipt and use same collect application form.
