Course of Study: Professional Diploma in Health Information Management
Duration: Three (3) Years
Entry Qualification:
* Five credits in O’level at not more than two sittings at SSCE/WASSC or its equivalent.
* The subjects must include English Language, Mathematics, Physics or Chemistry and any other subject.
Certification: DIPLOMA IN HEALTH INFORMATION MANAGEMENT (HRORBN)
Course of Study: ND in Health Information Management Duration: Thwo (2) Years
Entry Qualification:
* Five (5) O’level credits in English Language, Mathematics, Chemistry, Physics and Biology or Health Science in either WAEC/NECO at not more than two sittings.
* Only candidates with JAMB score of 150 and above are eligible.
Certification: NATIONAL DIPLOMA IN HELATH INFORMATION MANAGEMENT
Course of Study: HND in Health Information Management
Duration: Thwo (2) Years
Entry Qualification:
* National Diploma (ND) in H.I.M with a minimum of Lower Credit.
* Health Records Technician Certificate with a minimum of Lower Credit and five (5) O’level credits in English Language, Mathematics, Chemistry, Physics and Biology or Health Science in either WAEC/NECO at not more than two sittings.
* Candidates must have completed one year industrial attachment. ND holders with initial JAMB score of 150 are not required to write JAMB.
* However, candidates without ND certificate are required to have a JAMB score of 150 and above.
Certification: HIGHER NATIONAL DIPLOMA IN HEALTH INFORMATION MANAGEMENT
Note: only 2017 JAMB scores will be accepted.
To access the online application form, candidates should follow the steps listed below:
1. Walk into any branch of First Bank Nigeria and pay the sum of N8,000.00 (Eight Thousand Naira) only through the College of Health Pay Direct platform.
2. Obtain payment acknowledgement slip indicating a unique payment ID from the bank after payment.
3. Visit the college website at www.chtcalabar.edu.ng after 3 - 4 hours and log on to the e-application portal with the receipt number and serial number indicated on the payment printout.
4. Print out a copy of their application form after registration.
Note:
* Closing date for sales of form is 3rd June 2017. The portal will be shut by midnight on 3rd June 2017.
* Applicants will be subjected to an entrance examination scheduled to hold on the 10th June, 2017 at the college compound.
* Candidates for HIM–ND, HIM–HND, EHO–ND and EHO–HND are exempted from the examination.
