Applications are invited from suitably qualified candidates for admission into the school of medical Records for the following programmes 

PROGRAMMES 

A. NATIONAL DIPLOMA

2. years programmes leading to the award of National Diploma in Health Information management (HIM)

ENTRY QUALIFICATION

5 credit level passes in not more than two sitting of the WASC  or NECO or GCE in Mathematics, English Language, and any three of biology,Chemistry, Health Science, Physics.

B. HIGHER NATIONAL DIPLOMA

2 years programme leading to award of Higher National Diploma in Health Information Management (HIM).

ENTRY QUALIFICATION

5 credit level passes in not more than two sitting of the WASC or NECO of GCE O' level Examinations in Chemistry, Health Science, Physics, In addition, prospective applicants should possess any of the following 

i. National Diploma in health Information management with passess at not lower than lower Credit and have completed one years of industrial work Experience in a recognized Hospital 

ii. Health Records Technician certificate with minimum of lower credit pass, one year of industrial Work experience in a recognized hospital and minimum of five years working experience

c. METHOD OF APPLICATION

Candidates should pay a non-refundable fee of N5,000.00 (five thousand naira) only to STERLING BANK (UPTH Branch) A/C No. 008832930 and present the teller to the cashier at the office of the Assistance Director, (Finance) for the issuance of an official receopt and on presentation of evidence of such payment, obtain an Application form from the office of the Assistant Director of Administration, Clinical Services & Training.

CLOSING DATE FOR SUBMISSION OF APPLICATON FORMS Completed application forms with photocopies of all relevant documents are to reach the office of the Assistant Director of Administration, Clinical Services & Training before 28th February, 2014.

SELECTION INTERVIEW

E. ENTRANCE EXAMINATION will hold in the school of post Basic Nusing Class Room 2 UPTH on March 6th 2014 at 10.00am prompt.

Candidates are to bring along ariginal copies of their certificates and writing materials. Mabile Phones are not allowed into the examination hall.

Please note that Candidates are to make necessary arrangements for their accommodation and feeding throughout the duration of the interview.
