Admission into University of Portharcourt Teaching Hospital Community Health Officers Program

Applications are invited from suitably qualified candidates for admission into 2 (two) years Community Health Officers Training Programme in the University of Port Harcourt Teaching Hospital, Port Harcourt, leading to the award of a Higher Diploma in Community Health.
ENTRY QUALIFICATION
Eligible candidtes must be physically and mentally fit and possess any following requirements:
1. Diploma in Community Health with not less than two years post-qualification experience plus 5 (five) credit level passes in SSCE (WAEC, NECO or GCE O/L) at not more than two sittings, which must include, English Language, Mathematicas and Biology/Health Science.
2. Community Health Extension worker with five years experience, 5 (five) credit level passes in SSCE (WAEC, NECO or GCE O/L) at not more than two sittings which must include:English Language, mathematics and Biology/Health Science.
3. Applicants must be registered with the Community Health Practitioners Registration Board of Nigeria and possesses a valid practice license.
METHOD OF APPLICATION
Applicants should pay a non-refundable fee of N5,000.00 (five thousand naira) into UPTH account No. 0008832954 with the Sterling Bank, UPTH Branch and obtain receipts from the office of the  Assistant Director (Finance). They should on presentation of evidence of such payment obtain Forms from the Administrative Office, Community Health Officers programme Complex from Monday, 12th May, 2013 to Friday, 13th June, 2014 Duly completed application forms should be returned with two passport size photographs, photocopies of educational and professional certificates on or before Monday, 16th June, 2014.
Entrance examination would be conductedon Wednesday, 18 June, 2014.while the date for Oral interview would be given at the venue of the entrance examination
