Applications are hereby invited from suitably qualified candidates for admission into the Community Health Officers Training Institution, University of Ilorin Teaching Hospital for the 2015 Academic Session

Course Duration: 2 years Higher Diploma (H.D.) Programme.

This application form will not be accepted unless Part ‘B’ is completed by your Employer/Sponsor and Accompanied with Application Fee of N5,000.00 Payable into UBA PLC, CHO A/C NO: 1002005941, Branch: Muritala Mohammed, Ilorin.

Note: Examination is both written and Oral

Examination Date: 25th – 29th May, 2015

This date is subject to change at the discretion of the Management.

The applicants should send completed forms to the school either personally or by courier on or before 25th May, 2015.

All Original certificates/statement of results must be presented during the Oral examination as listed above.

(i) For the Written Examination, come with ALL your written materials i.e. Biro, Pencil, Ruler, Eraser, Calculator etc.

(ii) Come along with:

• Last Promotion letter

• Letter of Nomination by your employer for the course

• Marriage certificate or change of name (where necessary)

• Birth certificate or declaration of age (where necessary

• Letter of Attestation

• Letter of Sponsorship

• Any other relevant document(s) to the course

• Completed forms must be submitted to the school.
