Community Health Officers Training Program Forms on sale at University College Hospital Ibadan

The Community Health Officers’ Training Programme was established on the 15th October, 1979 for the purpose of training Community Health Officers who will be able to function effectively in providing Primary Health Care Services at the Local Government Area Level.
 
Applications are invited from suitably qualified candidates for admission into the Community Health Officers’ Training Programme, University College Hospital, Ibadan for the 2016/2017 session beginning in September, 2016.
 
Candidates who attain acceptable standard at the entrance examination will be considered for selection following interview.
 
The course is for a period of two years of four semesters after which candidates will bepresented for the National Final Qualifying Examination for the award of Higher Diploma in Community Health.
 
a. Holders of Diploma in Community Health with not less than (2) two years post qualification experience plus five (5) credit level passes in SSCE (WASCE, NECO OR GCE O/L) at not more than 2 sittings, which includes English Language, Mathematics,Biology/ Health Science, Chemistry, Physics.
b. Community Health Extension Workers with 5 years experience plus five (5) credit level passes In SSCE (WASCE, NECO or GCE O/L) at not more than 2 sittings which must include English Language, Mathematics, Biology/Health Science, Chemistry and Physics 
c. Applicants must be registered and possess valid practice license
 
METHOD OF APPLICATION 
Interested candidates are expected to pay a non-refundable fee of N7,500.00 (Seven Thousand,Five Hundred Naira) only to Community Health Officers’ (CHO) TSA and present the teller to the school for receipt and collection of the admission form.
 
SALE OF FORMS DEADLINE
Sale of forms for the 2016/2017 Academic Session commences on Monday, 8th February, 2016 and would last till 20th May, 2016.
Completed application forms must reach the school on or before 31st May, 2016.
