Lagos University Teaching Hospital, 2016 Admission into Community Health Officers' Course

Applications are invited for admission into the Community Health Officers' Coursefor2016/17 session. which will commence in August/September, 2016. 
 
1. ENTRY .QUALIFICATION
 
(A) Holders of Diploma in Community Health with not less than two years post­ qualification experience plus. five (5) credit level passes in SSCE (WASCE. NECO or GCE OIL) at not more than two sittings, which must include English Language. Mathematics and Biology/Health Science. 
(B) Community Health Extension Workers with 5 years experience and five (5) credit level passes in SSCE (WASCE, NECO or GCE OIL) at not more than two Sittings, which must include: English language. Mathematics and Biology/Health Science. 
(C) Applicants must be registered with the Community Health Practitioners' Registration Board of Nigeria and possess a valid practice license. 
 
2. DURATION OF COURSE: 
 
This is a two (2) year Residential Course, leading to the award of Higher Diploma in Community Health by the Community Health Practitioners Registration Board of Nigeria and the Lagos University TeaChing Hospital. 
 
3. LOCATION OF COURSE
 
Most activities connected with the course. including lectures, will take place at the LUTH Primary Health Care Complex at Pakolo (lfo Local GovemmentArea of Ogun state) and the surrounding villages, except when it is necessary for students to go to LUTH.ldi -Araba, Lagos. for lectures. 
 
4. METHOD OF APPLICATION: 
 
Application letter with photocopies of credentials and two passport sized photographs should be addressed to:

and should be accompanied with the receipt ofthe applicaton fee of N5,000.00(Five thousand naira). Fees are payable in cash to the Gash office ofthe Lagos University Teaching Hospital, Idi -Araba. 
 
5.CLOSING DATE FOR APPLICATION: 
 
Closing date for application is Friday, 27th May, 2016. Test and interview will be conducted on Thursday, 2nd and Friday, 3rd of June, 2016, in the lecture Theatre Block of the CHO School. LUTH PHC Complex. Pakato, Ogun state. 
 
Further invitation to attend interview wlll not be issued. 
 
Candidates should bring to the interview, originals of their credentials, letters of sponsorship from their Local Govemment/lnstitution and also Attestation letters from their employers stating that they will practice Primary Health Care at the end of the course. Successful applicants will be informed subsequently.
