Notice is hereby given that the final examiniation for the Diploma in Family Medicie will take place from January 13-15, 2016 as indicated below:
Date: Wednesday January 13, 2016: Paper 1- MCQ Venue: St. John Ambulance Training Hall, LUTH, Idi-Araba, Lagos
Time: 9.00am to 12 noon
Date: Thursday 14th January 2016: Paper II -Clinical Examination
Venue: Department of Family Medicine(Staff Clinic), LUTH, Idi-Araba, Lagos
Time: 8.00am Onwards.
The Application form for the examination can be downloaded from the Faculty of Family Medicine website: www.fammed.org.ng or at the zonal lecture centres.
GENERAL INFORMAITON:
This examination is only for those who have gone through the required training of the Diploma Programme.
(a) Each candidate must complete the application form fully correctly and honestly. ANy false information wil automatically disqualify the candidate.
(b) Application submitted after the closing date or incorretly/incompletely filled or not accompany with the appropriate information will also not be processed.
STEPS IN APPLICATION
Intrnding applicant should pay a non-refundable application fee of N100,000.00(One hundred Thousand naira only) to the National Postgraduate Medical College of Nigeria through the Treasury Single Account Platform by taking the following Steps:
A. Log on to www.remita.net. B Click "Pay a Federal Government Agency" Link; C Enter " National Post"; D Click on National Postgraduate Medical College of Nigeria; E. Select Service/Purpose.(Others); F. Fill Other details; G. Click proceed to payment; H. Select how you want to pay e.g ATM Cards; Bank Branch etc; I Complete Payment; J Print RRR receipt;
After payment send copy of the RRR receipt along with duly completed and signed form to either the zonal lecture centre where you were registered or
a. Jos zonal Coordinator (Abuja Centre): Dr. S. Yohanna Bingham University Teaching Hospital Jos.
b. Awka zonal Coordinator: Dr. N.P Obiegbu, Mezie Hospital, Upper New Market Street, Onitsha.
c. Lagos zonal Coordinator: Dr. A. J. Ariba, Dept of Family Medicine, Olabisi Onabanjo University Teachng Hospital, Sagamu.
No Cash, Bank Drafts or Personal Cheques will be Accepted.
Submission of incomplete form/documents shall lead to disqualification and College shall not enter into any correspondence with such applicants.
Closing date for the submission of forms is Friday December 31, 2015
