Notice is hereby given that the final examination for the diploma in family medicine will take place on 13h and 14th January 2015 2015 as indicated below:

 

TUESDAY 13TH JANUARY 2015: MCQ Paper, venue: college auditorium NPMCN, ijanikin

WEDNESDAY 14TH JANUARY 2015: Clinical Examination, Venue: Dept of family medicine, luth idi araba

 

The application form for the examination can be downloaded from the faculty of family medicine website www.fammed.org.ng

 

GENERAL INFORMATION:

This examination is only for those who have gone through the required training of the diploma programme

a) Each candidate must complete the application form fully, correctly and honestly, any false info will automatically disqualify the candidate

b) Application submitted after the closing date or incorrectly/incompletely filled or not accompanied with the appropriate info will also not be processed.

STEPS IN APPLICATION

1) Intending applicant should pay a non refundale application fee of N100,000 direclty through funds transfer or ATM into the college account through any branch of zenith international bank plc

account name: college account (NPMCN), Account number 1010437206, Bank sortcode: 057150068

 

11) Original copy of the confirmation of payment (not photocopy) should be attached to the form.

111) Obtain and submit the completed application form at either the zonal lecture center where you were registered or

NO CASH, BANK DRAFTS OR PERSONAL CHEQUES WILL BE ACCEPTED.

submission of incomplete forms/documents shall lead to disqualification and college shall not enter into any correspondence with such applicatns

Closing date for the submission of forms is friday december 31 2014
