NOTICE TO ALL PENSIONERS
VERIFICATION EXERCISE
This is to inform All Pensioners in the Obafemi Awolowo University Teaching Hospitals Complex.lle-Ife that the Institution will conduct a verification exercise for all Pensioners under PPVE ie. Pensioners who retired under the old scheme between 10th
February and 21th February, 2014
The Verification Exercise will take place at the following centres:
1. ObafemiAwolowo University Teaching Hospitals,lfe Hospital Unit lIe-lfe, between 10th and 14th February, 2014.
2. Wesley Guild Hospital,Ilesha between 1th and 2151 February, 2014.
Each Pensioner is advised to come with the underlisted documents:
1. First letter of ApPointment.
2. Letter of Confirmation of Appointment
3. Letter of last promotion
4. Letter of retirement
5. Pensioner Identity Card
6. One recent Passport Photograph
7. Bank Details (Bank name, Branch and New 10 DigitAccount Number)
The exercise will take place at the following Venues:
(i) Ife Hospital Unit - Archdeacon Ogunlade Cafeteria
(ii) Wesley Guild Hospital - GOPD Seminar Room
All affected Pensioners are hereby advised to ensure that they present themselves physically for the exercise at the Centre that is nearestto them.
Please, note that failure to attend the Verification Exercise would result in stoppage of payment of monthly pension allowance.
