This is to inform the general public that Admission Form for Entrance into the School of Basic Midwifery, Adazi-Nnukwu is now on sale.
Requirements: Candidates must have CREDIT in WAEC/NECO in the following subjects in not more than two sittings.
A. English Language
B. Mathematics
C. Biology
D. Chemistry
E. Physics
Method otApplication:
Admission form is obtained from the School on payment of a non-refundable fee of five thousand, four hundred naira only (N5,400)
Submission:
The Entrance form is submitted to the School with the followings:
A. Two (2) coloured passport photographs
B. Birth Certificate/Declaration of Age
C. Photocopy of WAEC/NECO Certifcate or Computer Print-out
D. Testimonial (Secondary School)
E. NECO/WAEC Scratch card(s) as the case may be
Entrance Examination:
The entrance examination is scheduled as follows:
Date: 10th December, 2016
Time: 8am prompt
Venue: School Premises
Note:
Candidates are expected to come to the exam hall with the evidence of payment (the receipt) for the admission form.
