OBAFEMI AWOLOWO UNIVERSITY TEACHING HOSPITALS COMPLEX 

SCHOOL OF HEALTH INFORMATION MANAGEMENT 2014/2015 ADMISSION

Applications are invited for admission into the School of Health Information Management for a 2 year programme leading to the award of the National Diploma (ND) and the 2 year programme leading to the award of the Higher National Diploma (HND) of the Health Records Officers Registration Board of Nigeria (HRORBN). The programme is to commence in September. 2014. 

1. ENTRY QUALIFICATION

Candidates seeking for ND admission must have obtained the minimum entry requirement of five (5) credits at not more than two sittings in the WASC/SSCE/NECO in the following subjects, 

a. English Language 

b. Mathematics 

c. Biology and any other two subjects such as Physics. chemistry. Agric Science, Geography and Economics. 

d. Candidates who sat for the year 2014 UTM and possess the above entry qualifications with not less that 150 total scores and who also have flair for Health Information Management can also apply. 

e. Candidates intending to pursue the 2-year HND programme must have obtained in addition to 

5 credits at O'levels, the National Diploma Certificate (not below Lower Credit) in Health Information Management. together with an evidence of completion of one year (compulsory) Industrial Attachment in areputable health institution. 

f. Possession of the National Certificate in Health Records obtained from a reputable institution in addition to the above requirements will bean added advantage (as required by NBTE) 

g. Pregnancy is not allowed during training 

h. Selected candidates must undergo and pass medical examination. 

2. CONDITIONS FOR ADMISSION

Selection of candidates will be based on the possession of requisite academic qualification, good performance in both the written examination and oral interview. The written examination for NO applicants will be tested in: 

a. English Language 

b. Mathematics 

c. General paper 

For the HND programme, applicants will be tested in English Language. Mathematics, Health Records Management. Statistics and Knowledge of Medical Terminologies. Selected candidates will be required to pass the medical fitness examination. 

METHODS OF APPLICATION 

Interested candidates are to pay a non-refundable fee of N5,OOO to the School Account Number 6000024658 at Mainstreet Bank OAUTHC, 1Ie-lfe, on presentation of the receipt for payment, application forms are to be collected from the office of :

Completed application forms must be returned to the HEAD OF SCHOOL not later than 15th July, 2014. Entrance Examination for all applicants comes up on Thursday 31 st July, 2014. in the school's premises by 9.00arn
