SEPTEMBER 2014 PRIMARY FELLOWSHIP EXAMINATION

The SEPTEMBER 2014 Primary Fellowship Examinaton of the College will hold in the underlisted Specialties (Faculties) on WEDNESDAY 24th SEPTEMBER 2014. Candidates should please indicate boldly at the top of their appucanon forms their centre of preference, NYSC Discharge Certificate is not a Mandatory requirement for Eligibility for the Primary Examination

There are eight (8) Centres for the Primary Examination viz: 

Abuja - National Centre for Women Development, Tafawa Balewa Street. Central Business District, Garki 

ii) Benin - Oba Akenzua Complex, University of Benin Teaching Hospital.(UBTH), Benin 

iii) Calabar - Conference Hall, Permanent Site of University of Calabar Teaching Hospital. Calabar. 

iv) Enugu - University of Nigeria Teaching Hospital (New Site) Ituku Ozalla Enugu lIe-lfe - Paediatrics Surgical Lecture Theatre. Obaterru Awolowo University Teaching Hospital Complex,(OAUTHC) lIe-lfe 

vi) Ilorin - College Lecture Theatre Complex. University of Ilorin teaching Hospital Oke Ose, Ilorin 

vii) Lagos - NPMCN Examination Halls, Km 26 Lagos-Badagry, Expressway. Ijanikin. Lagos. 

viii) Owerri - Nigerian Medica! Association {NMA) Secretariat off Concorde avenue off Port- Harcourt Road, New Owerri, Owerri. 

II. EXAMINATION FEES 

PRIMARY EXAMINATION FEES IS FORTY THOUSAND NAIRA ( 40,000.00) ONLY FOR ALL FACULTIES. 

III. APPLICATION FORMS AND PROCEDURES FOR ENTERING FOR THE EXAMINATIONS

IV. MODE OF PAYMENT (CASHLESS)

(a) Full Payment should be made DIRECTLY into the College Accounts through BANK DRAFT, FUNDS TRANSFER OR ONLINE WITH ATM Cards. Original copy of the confirmation of payment (not photocopy) should be attached to the form and returned to the College All candidates should write their names, address and Faculty and inscribe PRIMARY at the back of the BANK SLIP for confirmation of payment or PRINT-OUT of confirmation for online payment. Thereafter, official receipts of the College will be issued. 

CASH AND PERSONAL CHEQUES ARE NOT ACCEPTED FOR PAYMENT TO OUR ACCOUNTS 

(b) CANDIDATES MUST PAY INTO THE ABOVE STATED ACCOUNTS. ANY CANDIDATE WHO PAYS INTO ANY UNAUTHORISED ACCOUNT OR TO AN

UNATHORISED PERSON WIll BE DEEMED NOT TO HAVE REGISTERED FOR THE EXAMINATION AND WILL BE TREATED ACCORDINGLY. BANK DRAFTS FORWARDED WITH FORMS DIRECTLY TO THE COLLEGE WILL BE RETURNED, 

CLOSING DATE: 

Completed application forms accompanied by all relevant documents should reach the Deputy Registrar (Academic) at the College address not later than THURSDAY 31" JULY, 2014. Please remember to post-code your self-addressed and stamped envelopes. 

VI. CAUTION TO AVIOD DISQUALIFICATION 

(a) Applicants are advised to familiarize themselves with the requirements of their respective Faculties for entry to sit for the examinations, The submission of incomplete forms/documents shall lead disqualification and College shall not enter into any correspondence with such applicants

(b) NO CHANGE OF FACULTY OR CENTRE Will BE ALLOWED AFTER THE SUBMISSION OF APPLICATION FORM FROM ANY PRIMARY CANDIDATE. 

(c) ATTESTATION/RECOMMENDATION OF CADIDATES

Only Fellows who are in good financial standing With the College are eligible to attest to applications and recommend candidates for the College examinations. 

VII. LATE ENTRY 

Applications received after the closing date will be treatec as late entry. A late entry fee of Twenty thousand naira (N20,000.00) only per candidate will be charged. Submission of die forms will be allowed for a period of two (2) weeks only from the orgiral closing date. Applications received after the late entry deadline will NOT be precessed fer me currentexamination
