The September/October/November 2014 Part I Examinations of the College will holds as fellows
THE FOLLOWING FACULTIES WILL CONDUCT THE TWO-STAGE PART I FELLOWSHIP EXAMINATIONS IN SEPTEMBER/OCTOBER/NOVEMBER 2014:
ANAESTHESIA, FAMILY MEDICINE, INTERNAL MEDICINE, PAEDIATRICS, PATHOLOGY, PUBLIC HEALTH, RADIOLOGY, OTORHINOLARYNGOLOGY AND
SURGERY
1.The FIRST STAGE of Part I Fellowship Examination in these 9 Faculties will hold on THURSDAY SEPTEMBER 25th 2014 in THREE CENTERS namely
(a) Abuja - National Centre for Women Development, Tafawa Balewa Street, Central Business District, Garki
(b) Enugu - University of Nigeria Teaching Hospital (New Site), Ituku Ozalta Enugu
(c) Lagos - NPMCN Examination Halls, Km 26 Lagos-Badagry Expressway, Ijanikin, Lagos.
THIS IS A MULTIPLE CHOICE QUESTIONS (MCQS) TYPE OF EXAMINATION.
ii. ONLY CANDIDATES WHO PASSED THE FIRST STAGE OF THE
EXAMINATION WILL BE INVITED FOR THE REMAINING PHASES OF THE EXAMINATION.
iii. EXAM FEES: Pathology N80,000.00, Others N60,000.00 Candidates in these F-aculties are to download an updated Part 1 Examination Form from the College website and Choose ONE of the THREE CENTERS NAMELY: ABUJA, LAGOS and ENUGU, indicated on the top of the application form
iv. Candidates should clearly and correctly fill in their e-rnau addresses and telephone numbers in addition to other details required in their application forms as Notification Letters for Eligibility to sit for First Stage (MCQ) Part 1 Examinations and Notification of SUCCESS in First Stagellnvitation for other Parts of the Examination will be sent by email and text messages.
v. Candidates are to Print the Notification Letters specified in (v) above (as applicable to them) and affix a passport photograph each (same as used 'on Application Form) and bring to First Stage and Subsequent Phases of the examination for Accreditation purposes. List of SUCCESSFUL CANDIDATES in these nine Faculties will be uploaded on the College Website and displayed on Notice Boards in the College, and its liaison offices LUTH, (Lagos) and Abuja; as well sent to successful candidate's email addresses.
(viii) Candidates should endeavor to check the College website weekly as any new information will be uploaded.
(ix) SECOND STAGE OF PART I: Candidates who are successful in the FIRST Stage will proceed to sit for SECOND STAGE consisting of written, Clinical/Practical and Orals in the usual format.
2. PART I FELLOWSHIP EXAMINATIONS IN OTHER FACULTIES, NAMELY DENTAL SURGERY, GENERAL DENTAL PRACTICE, OBSTETRICS & GYNAECOLOGY, OPHTHALMOLOGY, PSYCHIATRY AND QRTHOPAEDICS WILL HOLD IN OCTOBER/ NOVEMBER 2014.
All candidates in the above faculties would sit for all parts of the Examinations Written (MCQfEssay), Clinical/ Practical/Orals as applicable to them and in the usual format,
3. PART II FELLOWSHIP EXAMINATIONS IN ALL FACULTIES WILL HOLD IN OCTOBER/NOVEMBER 20141N THE USUAL FORMAT.

EXAMINATION FEES
Fellowship Examination Fees in all Faculties are as indicated below:
a. PART I: FEES FOR FACULTY OF DENTAL SURGERY IS (N50,000.00) ONLY. FEES FOR OTHER FACULTIES ARE (H60,000) ONLY
b. PART II: FEES FOR ALL FACULTIES IS (H100,000.00) ONLY .

a. Full Payment should be made DIRECTLY into the College Accounts through
BANK DRAFT, FUNDS TRANSFER OR ONLINE WITH ATM Cards. All candidates should write their names, Address, Faculty and the Part of the fellowship examination that they intend to sit for at the back ofthe BANK SLIP for confirmation of payment or PRINT-OUT of confirmation for online payment. Thereafter, official receipts of the College will be issued on submission with forms filled. CASH AND PERSONAL CHEQUES ARE NOT ACCEPTED FOR PAYMENT TO OUR ACCOUNTS
b. CANDIDATES MUST PAY INTO THE ABOVE STATED ACCOUNTS. ANY CANDIDATE WHO PAYS INTO ANY UNAUTHORISED ACCOUNT OR TO AN UNATHORISED PERSON WILL BE DEEMED NOT TO HAVE REGISTERED FOR THE EXAMINATION AND WILL BE TREATED ACCORDINGLY. BANK DRAFTS FORWARDED WITH FORMS DIRECTLY TO THE COLLEGE WILL BE RETURNED.
ASSPCIAIEFE FELLOW'S DUES
Candidates are to ensure that all outstanding Associate Fellows' dues of five thousand naira (N5,000.00) annually with effect from 2010 are paid on or before 31st January 2014 for all College Examinations holding in 2014. Please note that this payment is ANNUAL irrespective of whether an examination will be taken or not. Late payment beyond this date will attract a penalty of five thousand naira (N5,000.00) annually per candidate. PLEASE NOTE THAT PAYMENT IS DIFFERENT AND SHOULD BE MADE ONLY INTO THE FELLOWS ACCOUNT WITH ZENITH BANK PLC, AGUDA BRANCH, LAGOS: ACCOUNT NO: 1010778138, SORT CODE 057150437 in the approved CASHLESS manner.

Completed application forms accompanied by all relevant documents should reach the Deputy Registrar (Academic) at the College address not later than THURSDAY 31th JULY, 2014. Please remember to post-code your self-addressed and stamped
envelopes.
9. CAUTION TO AVOID DISQUALIFICATION
(a) Applicants are advised to familiarize themselves with the requirements of their respective Faculties for entry to sit for the examinations.
The submission of incomplete forms/documentsshall lead to disqualification and College shall not enter into any correspondence with such applicants.
(b) ATTESTATION/RECOMMENDATION OF CANDIDATES
Only Fellows who are in good financial standing with the College are eligible to attest to applications and recommend candidates for the College examinations.
OTHER INFORMATION
(a) ANNUAL PROGRESS REPORT
An Annual Progress Report on Registered Residents/Associate Fellows of the College is a MANDATORY requirement for the College Examination. The annual progress Report form should be downloaded from the College website, completed ane forwarded to the College Registrar on or before January 31st of each year for a report on the preceding year. Reports on 2013 for all Residents/Associate Fellows should have been forwarded by the Training Institutions on or before 31st January, 2014. A minimum of two reports for the two previous consecutive years is a prerequisite for all Parts I & II Examinations as from January, 2014. Thus all Registered Residents/Associate Fellows MUST have submitted their annual progress report on or before 31th January, 2014 for examinations in 2014.
Please note that completion and submission of this form is an annual event irrespective of whether an examination will be taking or not during the year.
(b) DISSERTATION/RESEARCH PROPOSALS
For Part II examination, Candidates must submit four (4) copies of their Research Proposals for Formative Assessments at least 15 Months before intended examination. Please note that Candidates are only eligible to apply for the Part II examinations at least 12 Months after APPROVAL of the Formative Assessments. Each proposalloosely/spiral bound must be accompanied with a processing fee of Twenty thousand, naira (N20,000.00) only.
(c) OBSTETRICS & GYNAECOLOGY PART 1 CASEBOOK
All candidates applying for Part I Obstetrics and Gynaecology must submit their qualifying books -with an assessment fee of Twenty thousand naira (N20,000.00) only at least 8 months before application for the intended examination closes. Only Candidates with approved case books are eligible for examination.
(d) EXEMPTION FROM PRIMARY EXAMINATION
All candidates applying for Exemption from the Primary Fellowship Examination must pay an exemption fee of One hundred thousand naira (N1 00,000.00) only.
A copy of original confirmation of payment should be attached to the form and returned to the College. Only candidates with equivalent qualification less than five years need apply. Other requirements are on the College website. .
Payments should be made directly into the College Examinations Account designated banks above via funds transfer/bank drafts/ATM cards.

a. The 8th Annual Scientific Conference and All Fellows Congress of the College will hold in UYO, Akwa Ibom State from 12th - 16th August, 2014
