University of Benin Teaching Hospital, Benin City has a Mono-technic accredited by relevant Regulatory bodies JNationa/ Board for Technical Education (NBTE), Health Records Officers Registration Board of Nigeria (HRORBN) and the Joint Admissions Matriculation Board / Unified Tertiary Matriculation Examination (JAMB/UTME) to train health professionals in Health Information Management.

•  Candidates are to pay a non-refundable fee of Five Thousand, Five Hundred Naira {N5, 5,000) only; payable into the Schools' Account with (Fidelity Bank, "Corporate Patients' Account";Acct. No: 5030047475

Completed forms should be returned to the Office of the Head of School on or before Friday 21st August, 2015.
