Applications are invited from suitably qualified candidates for admission into a two-year Community Health Officers Training Programme leading to the award of Higher Diploma in Community Health in the above named School for the 2016/2017 academic session.
ENTRY REQUIREMENTS:
1. BASIC EDUCATIONAL QUALIFICATION: WASC/GCE 0/L/SSSC/NECO/NABTEB O/L in five (5) credits (in not more than two sittings) in the following areas: English Language, Mathematics, Biology or Health Sciences and ONE each from the two groups viz: Group A: Chemistry, Physics, Geography, Food and Nutrition and Group B: Economics, Accounting, Government, Commerce, Agricultural Science, Literature in English, Book Keeping.
2. PROFESSIONAL QUALIFICATIONS AND EXPERIENCE: i. Diploma in Community Health (DCH) with not less than 2-year post-qualification experience
ii. Community Health Extension Workers (CHEW) Certificate with not less than 5-year post-qualification experience
3. Registration with the Board: Applicants must be registered as a Community Health Practitioner and possess a valid practice license.
4. DATES FOR WRITTEN TEST AND ORAL INTERVIEW: Wednesday, 20th and Thursday, 21st July, 2016 respectively. (Candidates are to bring along their original and photocopies of their certificates/master-lists to the examination centre).

a. Application forms are available at the Office of the Principal in UBTH Benin City/downloaded from UBTH website: www.ubth.org or obtained at the interview venue with evidence of payment of prescribed fees. Further enquiries should be directed at Adediji Adewale, 08023344019.
Completed forms should be returned to the undersigned with evidence of payment of Ten Thousand Naira (#10,000.00) only made payable to UBTH TSA (Service Type: Training Schools) via REMITA platform in any bank in Nigeria on or before July 18, 2016.
b. Scanned receipt from the bank should be sent to the school e-mail address: [email protected]. Please make your full name and phone number the subject of the e-mail.
C. OTHER DOCUMENTS TO BE SUBMITTED: i). Scratch card(s) for SSCE/NECO/NABTEB results verification. ii) Three recommendation Letters. iii) Evidence of change of name (if applicable)
