Applications are invited from successful candidates who scored 150 and above in the 2015 UTME for admission into the School of Health Information Management for a Two-Year Programme leading to the award of the Ordinary National Diploma and Two-Year Programme leading to the award of the Higher National Diploma of the Health Record Officers Registration Board of Nigeria (HROBN). The Course shall commence in September 2015. The school is Non-Residential. 

 

1. ENTRY QUALIFICATION FOROND PROGRAMME 

Candidates must have obtained the minimum entry requirement of five (5) Credits at not more than two sittings in the GCE/SSCE/NECO in the following subjects: 

 

(a) English Language 

(b) Mathematics 

(c) Biology or Health Science, Physics or Chemistry and any other subject. 

 

Please note that: 

Possession of the National Certificate in Health Records obtained from a reputable Institution in addition to the above will be an added advantage.

ii. Pregnancy is not allowed during training 

iii. Selected candidates will be required to undergo and pass medical examination. 

 

 

CONDITIONSFORADMISSION 

Selection of candidates will be on the basis of good academic qualification, good performance in both the written examination and oral interview, and presentation of ORIGINAL CERTIFICATES. 

 

The written examination for ONDadmission will be in (i) English Language, (ii) Mathematics, (iii) General Paper whilst the written examination for HND admission will be in: 

(a) English Language 

(b) Mathematics 

(d) Health Records Management 

(d) Statistics 

(e) Medical Terminology. 

 

METHOD OF APPLICATION 

Interested candidates are advised to pay a non-refundable fee ofN5, 000 into the VCH SHIM Account No. 1013746811 at Zenith Bank International. 

 

Applicants should collect a receipt for the payment from the office of the Assistant Director (Finance & Accounts ), University College Hospital, Ibadan. 

 

The receipt and the photocopy of the bank teller should be forwarded

Completed Application Forms must be returned to the Head of School, not later than 14th August, 2015. 

 

Post-UTME for OND Applicants will be conducted on Saturday, 221id August, 2015, and that of HND Applicants on Monday, 31st August, 2015 in the School Auditorium by 9am. 

 

Successful applicants will be invited for an oral interview at a later date through text messages and School Notice Board.
