Applications are hereby invited from suitably qualified candidates for admission into the School of  Community Health Officers for 2015/2016 Academic session

Course Duration: 2 years Higher Diploma (H.D.) Programme.

NOTE:

This application form will not be accepted unless Part ‘B’ is completed by your Employer/Sponsor and Accompanied with Application Fee of N5,000.00 Payable to UITH SCHOOL COMPLEX ACCOUNT through REMITA.

Note: Examination is both written and Oral

Examination Date:  23rd – 27th May, 2016.

venue: Old Maternity Wing, UITH School Complex, Amilengbe, Ilorin.

Time: 9am Prompt.

This date is subject to change at the discretion of the Management.

The applicants should send completed forms  to the school either personally or by courier on or before 23rd May, 2016.

All Original certificates/statement of results must be presented during the Oral examination as listed above.

(i) For the Written Examination, come with ALL your written materials i.e. Biro, Pencil, Ruler, Eraser, Calculator etc.

(ii) Come along with:

•          Last Promotion letter
•          Letter of Nomination by your employer for the course
•          Marriage certificate or change of name (where necessary)
•          Birth certificate or declaration of age (where necessary
•          Letter of Attestation
•          Letter of Sponsorship
•          Any other relevant document(s) to the course
•          Completed forms must be submitted to the school.
