Admission into Community Health Officers' Training Program (C.H.O.T.P) for the Higher Diploma Program in Community Health

 

Application are invited from suitably qualified candidates for admission into the Community Health Officers' Training Program (C.H.O.T.P), University of Maiduguri Teaching Hosptial for the 2015/2016 academic session as from 04/05/2015 - 31/07/2015. 

 

MINIMUM ENTRY REQUIREMENTS:

A. Holders of diploma in Community Health with not less than two years post qualification experience, plus five (5) credits level passes in SSE (WAEC), NECO or G.C.E (O/L) at not more than two (2) sittings which must include English Language, Mathematics, Biology/Health Science, Chemistry/Food & Nutriton/Physics/Goegraphy, Economics/ Agric.Science/Accounts/Commerce

 

B. Communitty Health Extension Worker (CHEW) with five (5) years post qualification experience plus five (5) credits level passes in SSCE (WAEC), NECO or G.C.E (O/L) as mentioned in iteam (a) above.

 

C.  Applicants must be registered with the Community Health Practitioners' Registration Board of Nigeria (CHPRBN) and must possess valid and up to date practicing license

 

 

DURATION OF THE PROGRAM:

This is a two (2) year residential program leading to the award of Higher Diploma in Community Health 

 

METHOD OF APPLICATION 

Application forms are available in the office of the Coordinator, Community Health Officers' Training Program (CHOTP), University of Maiduguri Teaching Hospital, after a payment of non-refundable fee of N5.000:00 (five thousand naira only) to the UMTH Account No. 6000082010 at any Main Street Bank Plc, National wide.

 

The copy of the bank teller should be submitted to the UMTH Accounts department and a receipt will be issued in respect of such payment. The application form will only be issued to an applicate on presentaion of the original copy of the receipt issued by UMTH Accounts department.

 

Completed application forms should be returned to the Coordinator's office on or befor 07/08/2015

 

ENTRANCE EXAMINATION AND INTERVIEW

Only candidates who satisfy the basic entry requirements shall be invited for an entrance examination/interview from 10th - 14 Aug. 2015

 

NB: Preference will be given to candidates with evidence of guaranteed sponsorship by government agencies, i.e. federal, states and local government health workers
