Admission into National Diploma (ND) and Higher National Diploma (HND) Programme for 2015/2016 Academic year

 

a.  National Diploma (ND)

 

This is to invite all eligible candidates with the School Health Information Management UMTH as their first or second choice to Post UTME screening exercise. Those who wish to change their choice of course are also invited. Eligible candidates must possess SSCE/NECO or its equivalent with five (5) credits in relevant subjects (English, Mathematics, Biology Physic or Chemistry and any other one subject and JAMB aggregate score of not less than 150.

 

Date: 5th October, 2015

Time: 10:00pm prompt

Venue: School Auditorium

 

Interested candidates shoulf pay non refundable screening fee of three thousand five hundred naira (3, 500: 00) into the School Management Account at Sky Bank plc.

 

A/c no. 4090601470 and come along with the receipt/teller of payment to the screening venue.

 

 

B.  Higher National Diploma (HND)

Application form is available for all interested candidates for admission into two year HND programme in Health Information Management (HIM) with effect from 14th September, 2015. Candidates must possess National Diploma or Professional Diploma in HIM with minimum of lower credit and evidence of one year industrial attachment in any reputable health institution.

 

The form should be obtained in the school after payment of non refundable fee of five thousand naira (N5, 000.00) to the above mentioned account. Completed form (HND) must reach the admission office on or before 5th October, 2015
