APRIL 2014 PRIMARY, PARTS I & II (FINAL) FELLOWSHIP EXAMINATIONS

WEST AFRICAN COLLEGE OF SURGEONS
The April 2004 Fellowship Examinations of the West African College of Surgeons (WACS), in the Faculties of Anaesthesia, Dental Surgery, Obstetrics and Gynaecology, Ophthalmology, Otorhinolaryngology, Radiology and Surgery, will be held from 2nd to 14th April, 2014
THE PRIMARY EXAMINATIONS WILL BE HELD ON FRIDAY 4TH APRIL, 2014 IN FIVE CENTRES AS FOLLOWS:
University College Hospital (UCH), Ibadan, Nigeria
National Centre for Women Development (NCWD), Abuja, Nigeria
Korle-Bu Teaching Hospital (KBTH), Accra, Ghana
National Orthopaedic Hospital (NOH) Main Hall, Enugu, Nigeria
Connaught Hospital Freetown, Sierra Leone
The Parts I&II Examinations will hold from 2nd April 2014 (Obstetrics & Gynaecology) and 7th April 2014 (Anaesthesia, Dental Surgery, Ophthalmology, ORL, Radiology and Surgery) in Two Centres namely:
University College Hospital (UCH) Ibadan, Nigeria
Korle-Bu Teaching Hospital (KBTH) Accra, Ghana
REQUIREMENTS
Candidates applying to sit the Primary Fellowship Examinations must be Medical Practitioners who are fully registered in member countries, have acquired at least one year-post-registration experience. Candidates submit their degree certificates and evidence of FULL registration with Medical & Dental Council of their countries. Provisional Registration Certificate is NOT acceptable 
Candidates applying to sit the Part I Fellowship Examinatiopns must have passed the Primary Examination of the West African College of Surgeons or its equivalent and satisfactorily completed approiate years of Post graduate training as prescribed by the relevant Faculty in the institutions accrdited for the purpose by the college, Candidates are required to submit their original Certificants of Training (COT) form with dates of commencement and completion of training clearly stated, Altered or 
defaced COT will not be accepted,www.medicalworldnigeria.com. submission of photocopies of logbooks duly certified by their training consultants is commpulsory.
Candidates applying to sit the Part II (Final) Fellowship Examinations must have pssed the Part I Fellowship Examinations of the West Adrican College of Surgeons and satisfactorily completed appropriate years of Postgraduate training and worked in their specialty for at least Two (2) Years in an Institutions(s) accredited for the piurpose by WACS as prescribed by the relevant Faculty. Candidates are to submit their original Certificates of Training (COT) form with dates of commencement, 
completion of training and various postings/appointments, clearly stated, Altered or defacedCOT will not be accepted, Candidates are required to submit before 31st December 2013.
Candidates applying to sit O&G Part II Examinations must submit their Thesis dissertations and Case Books separately bound in Navy Blue Colour before 31st December,  2013 
Candidates applying to sit Surgery Part II Examinations must submit Three (3) Copies of their dissertation sepatately bound in spiral format and a copy of their resident's log-books at the time of submission of applications for the examination, They should in addition bring their file of raw results of the dissertation with them to the venue of the Oral examination.www.medicalworldnigeria.com
It is the candidate's responsibility to retrirve both his/her log-book and dissertation at the end of the examination. A copy of the disseration shall be kept by the College
Candidates applying to sit Part II Dental Surgery must submit Two (2) sopies of their logbooks while Candidates for Radiology and ORL Part II examinations are to submit a photocopy of their Logbooks, Three Copies of Approved Proposal Dissertations before the closing date of applications. Original copies of logbooks should be brought to the examination Venues 
All Part II candidates must submit an evidence of attendance of Manuscript Writing Workshop.

EXAMINATION FEES
1. Primary Examination          - N50,000.00  OR $315
2. Part I Examination           - N65,000.00  OR $410
3. Part II (Final) Examination  - 120, 000.00 OR  $750

For Candidates sitting examinations in Nigeria
All payments should be made at any
UNITED BANK FOR AFRICA PLC (UBA), into ACCOUNT NO.1014816816 ACCOUNT NAME "WEST AFRICAN COLLEGE OF SURGEONS".For Candidates sitting examinations in Accra, Ghana,payments should be made directly to ABRAHAM OKLETEY, WAPMC Liaison Office at UGMS, Korle Bu, Accra, Ghana.
For Candidates sitting examinations in Freetown, Sierra Leone, Payments should be made to WACS Country Representative, (Dr.A.D.O. Wright, 34 Bathurst Street, Freetown, Sierra Leone).
THE COLLEGE SHALL BEAR NO RESPOSNSIBILITY FOR PAYMENT INTO WRONG ACCOUNTS CASH PAYMENT IS UNACCEPTABLE.
Candidates must indicate their names in Teller Column 'Paid By'.
Candidates should write their Faculty and part of the Examination on the TELLER and submit same with their Examination 
Application Form to the College Secretariat either in person or by Courier Only

Current application forms are available at the Coollege Sercretariat in Yaba, Lagos and WAPMC Liaison Office at UGMS, Korle Bu, Accra, Ghana Application forms can also be downloaded from the College Website: www.wacs-coac.org.
ALL APPLICATION FORMS MUST HAVE ATTACHED "DOCUMENTSINCLUDED CONTROL FORM' WHICH COULD BE SOWNLOADED FROM THE WEBSITE ALONG 
WITH THE APPLICATION FORM AND RELEVANT DOCUMENTSDEFERMENT OF EXAMINATION AFTER SUBMISSION OF APPLICATION IS NOT ACCEPTABLE 
All Application forms must be submitted with:
Copy of Bank Teller - Primary/Part I/Part II
Copies of Dissertations and approved Proposals - Part II
Copies of Case Books - O&G Part II which must be submitted before January 31, 2014
Relevant certificates and photocopy of Log Books (Parts I & II)
Evidence of attendance at Manuscript Writing Workshop (Part II)
Evidence of training since passing Primary and Part I
Evidence of registration as Surgeon in trining (S.I.T) Part I only 
3 self-addressed enveloes with sufficient postage stamps 
CLOSING DATE
The closing date for the submission of completed application forms is January 3rd, 2014. Candidates for the Part II (final) Examinations in Anaesthesia, Dental Surgery, Ophthalmology, ORL, Radiology and Surgery must submit their disseratations by the closing date
PENALTY FOR LATE SUBMMISSION IS N30,000. All late applications close on January 10th, 2014. Applications and Dissertations submitted later than this date would not be processed.Candidates should indicate their e-mail addresses and their Telephone numbers on their application forms.
