2014 REGISTRATION PROCEDURE

The Pharmacists Council of Nigeria (PCN) wishes to inform all Pharmacists and other stakeholders involved in Pharmaceutical care service delivery that 

Registration/Renewal of Annual Licences, Premises Certificates, Annual Permits has commenced. Find below tile flow chart of the registration procedure for 2014 

FLOW CHART 

. Form and customized teller to be collected from the PS Office/ PCN State Office 

. Form to be completed and appropriate fees paid in the designated bank using the customized teller 

. Completed form and bank teller to be returned to . the DPS Office/ PCN State Office 

. PCN State Officer to forward completed forms to PCN Head Office/Registry 

. Processing and licensing at the PCN Head Office on receipt of forms from DPS Office/ PCN State Office 

. Licences/Certificates/Permits to be forwarded to DPS Office/ PCN State Officer 

. Licences/Certificates/Permits to be dispatched by DPS Office/PCN State Officer

Stakeholders are by this notice informed that early registration closes on march 31, 2014. Late registration attracts fifty percent of licencing fee as penalty
