The Community Health Officers Training School, UMTH wishes to inform the general public that the 2025/2026 Admission forms for Higher Diploma in Community Health are on sale. Application forms are therefore available for all interested candidates for admission into two (2) years and one (1) year Higher Diploma with effect from STH MAY to 30"H JUNE 2025.

 

ELIGIBILITY

For two (2) years training programme, candidates most have Diploma in Community Health (CHEW). Applicants should possess 5 credits in English, Mathematics, Biology, Physics and Chemistry.  

Candidates must have registered with the Community Health Practitioners Registration Board of Nigeria. The form should be obtain from the school after payment of non-refundable fee N5, 000.00 only and For One (1) year training programme, candidates must have Diploma Community Health (CHEW), BSc in Community Health and Health related courses in accredited University, must possess current practicing license from the Community Health Practitioners Registration Board of Nigeria, candidates with above certificate must possess five (5) credits in English, Mathematics, Chemistry Physics and Biology in SSCE/O Level in not more than two sittings.  

The price of the form is N10,000.00 only. All payment should be made through the UMTH account department.
