Job Description

We are seeking a compassionate and detail-oriented Care Coordinator to join our hospital team. 

The Care Coordinator will play a key role in ensuring seamless patient care by coordinating medical services, facilitating communication between healthcare providers, and assisting patients in navigating their treatment plans.

 

Key Responsibilities

• Act as a liaison between patients, doctors, nurses, and other healthcare professionals.
• Develop and implement personalized care plans to ensure continuity of care.
• Assist patients with scheduling appointments, referrals, and follow-up care.
• Educate patients and families about treatment options, medications, and healthcare resources.
• Monitor patient progress and collaborate with medical teams to adjust care plans as needed.
• Ensure compliance with hospital policies and healthcare regulations.
• Maintain accurate and up-to-date patient records and documentation.

 

Qualifications & Skills

• Bachelor's Degree in Nursing, RN,RM
• Previous experience in patient care coordination or healthcare administration is an advantage.
• Strong communication, organizational, and problem-solving skills.
• Knowledge of Nigerian healthcare systems and hospital procedures.
• Proficiency in medical record management and patient care software.
