• Evidence of payment of capitation fees, Building levies, to PSN, State Branches dues, and Technical/Interest Group dues for the past 3 years
• PSN ACCESS Bank Account number: 0693346616 not later than 31st May, 2021
• Copy of Bank Teller of N100,000.00 (One Hundred Thousand Naira) processing fee in respect of 2021 Fellowship Award
• Curriculum Vitae
• Current Annual Licence to Practice as a Pharmacist
• Any other relevant documents submitted

NOTE: All applicants must submit (12 sets of spiral bounded copies of their Application form/ Credentials/ CV/ Bank Teller in addition to necessary annexure for validations before the deadline of 31st of May 2021 to the PSN National Secretariat).
